Le CMF au Manitoba
In 2012, Manitoba saw the development of 4 Primary Care Networks, which are building on the work of Physician Integrated Networks (PINs) and its partnerships between regions, fee-for-service (FFS) clinics, and other partners, as well as incorporated aspects of coordinated, comprehensive primary care and focused on prevention and coordinated disease management.
In addition to this, Manitoba has committed to providing each Manitoban with access to a family doctor and works to providing My Health Teams and team-based care across the province. On April 9th, 2015 the MB College of Family Physicians and The College of Family Physicians of Canada hosed a PMH Symposium in Winnipeg titled Achieving the Patient’s Medical Home and the Manitoba Experience. The objective was to assist symposium participants in identifying ideas and next steps to improve implementation and promote uptake of PMH-like models in Manitoba and communities across Canada. The symposium was an opportunity to gain insight on what initiatives are working, what areas we can build on, and the challenges we should aim to address in order to realize the PMH vision in Manitoba.
