PMH teams ensure continuity of care is provided for their patients in different settings, including the family practice office, hospitals, long-term care and other community-based institutions, and the patient’s residence.
RECOMMENDATION #3
A PMH serves as the hub that ensures coordination and continuity of care related to all the medical services their patients receive throughout the medical community.
