The first pillar of the Patient’s Medical Home is patient-centredness, which means your family physician and other members of your health care team are responsive to your needs, preferences, and experiences. The team at your PMH will work to help you better understand areas of your health that need further attention and will seek your input on your care, to improve your health.
You will play a role in continuous quality improvement by providing feedback. The PMH encourages your participation in surveys that will help the practice evaluate its effectiveness and improve planning. By taking part on patient advisory councils or by completing surveys you can help the PMH achieve patient-centredness and provide better care for you.
