The College of Family Physicians of Canada™ has produced the Patient and Physician’s Experience in the Patient’s Medical Home to illustrate the key differences between Patient’s Medical Home (PMH) and non-PMH practices. This document aims to highlight the benefits of the PMH vision by presenting the experiences of a fictional patient and physician.
