PMH in British Columbia

The BC College of Family Physicians (BCCFP) believes the Patient’s Medical Home (PMH) is the future of family medicine in British Columbia.

In 2018, the provincial government announced the launch of a primary health care strategy with team-based care as the overarching principle and the PMH vision serving as the basis for Primary Care Networks (PCNs), which knit together services and organizations to better coordinate care for patients. As of 2022, 43 PCNs have been established across the province and six PCNs are in the final stages of planning.

There are some examples where the physician-led, team-based, patient-centred, continuous, comprehensive, and accessible vision for primary care outlined by the PMH has been realized, though opportunity remains to do more.

The BCCFP continues to advocate for the appropriate infrastructure and support necessary to adopt the PMH model as part of British Columbia’s primary care strategy.
