Patient’s Medical Neighbourhood Implementation Kits Now Available

The Patient’s Medical Neighbourhood describes a network of care involving multiple providers and services, with family practices as the central hub for coordinating care. To further support this model, we have created implementation kits for each province and the territories. 

The kits for British Columbia, Alberta, Saskatchewan, Prince Edward Island, Nova Scotia & Newfoundland and Labrador and the Northwest Territories, Nunavut, and Yukon in English only; kits for Quebec, Manitoba, Ontario and New Brunswick are available in French and English.
