The College of Family Physicians of Canada releases a practical guide on how family physicians can improve the social determinants of health.
A Patient’s Medical Home aims to provide each of its patients with a comprehensive scope of family practice services that also meets population and public health needs. Part of meeting these needs means that family physicians and other team-based providers should address the health needs of both the individuals and populations they serve, incorporating the effects that social determinants such as poverty, job loss, culture, gender, and homelessness have on health. A focus on these non-medical determinants of health is critical to improving the health of not just individual patients, but entire populations.
The College of Family Physicians of Canada strongly supports improving the social determinants of health for all Canadians as a matter of social accountability. The CFPC is pleased to provide family physicians with this Best Advice Guide.
The guide, a latest in the Best Advice Guide series, provides concrete steps for how family physicians and their teams can intervene to help improve the social determinants of health.
