Measurement, Continuous Quality Improvement, and Research

Your practice uses quality improvement (QI) techniques to measure and enhance practice effectiveness, health outcomes, and patient/provider satisfaction
The results of CQI are applied to your practice to enhance care, operations, services and programs
All members of your practice team (clinical staff as well as administrative support personnel), trainees and patients participate in your CQI process
Your practice supports all team members (physicians, other health professionals, students or residents) to initiate and carry out research projects
Your practice supports community-based research focusing on patient health outcomes and the effectiveness of care and services provided
Your practice has invested in formal or informal connections with other supports (ex: a PBRN, CPCSSN, SPOR Office, Health Quality Council, university-based Department of Family medicine, etc.) to support QI and research
Your practice has a mechanism in place to measure performance and track achievement of PMH goals
