Thank you for participating in the PMH Self-Assessment questionnaire. In order to complete the assessment you will be given a unique token (reference code) that will represent you as a unique individual.

While entering your email is not required, you will be able to return to re-take the self-assessment in the future, compare your current and past results and measure your progress.

You will not be able to compare multiple self-assessments over time without entering an email address.

If you do enter an email address, the self-assessment questionnaire will send you automatic reminders with the option to take a future self-assessment at regular, but infrequent intervals. Your email will not be saved or used for any purpose other than for use with this self-assessment questionnaire.
