PMH Talk Forum CMF
PMH Talk is a new forum available to everyone interested in the Patient’s Medical Home and will be an exciting new way to share, read, vote and comment on PMH news, developments and articles. PMH Talk has the potential to be a great collaborative space for physicians, patients, policy makers and other health care professionals to share knowledge and ideas about the Patient’s Medical Home model of care. It is a space for you to discuss, and as such relies on participation to thrive. Visit, post articles, comment on other items and like materials shared by others. And do so often! Let us know what you think by leaving a reply to this introduction, or even better, by getting the ball rolling by posting or sharing something of benefit to the Patient’s Medical Home.

Let’s Talk

It was an easy-to-read, informative piece. Check it out here: PMH Patient Flyer   Let me know what you think! Sally Hughes

PMH Practice Assessment
