What does it mean to be a champion?
Achieving and ensuring the success of the PMH vision is not an individual effort. Leadership can help build a strong foundation for the Patient’s Medical Home and is required across Canada to improve access to family medicine and health care. That’s where champions come in.
Champions will act as leaders, as points of contact, and as information disseminators. To be an effective PMH champion, you should understand the values and components of the PMH model, as well as what the PMH is not. You not only need to understand this information and the resources available, but know how it enhances the Canadian health care system. This is where our website can help. You can use the tools and information provided on this website to familiarize yourself with the PMH vision, as well as to share it with others.
You are championing a vision for better health. You, along with others who share the PMH vision, will help spread the word. You help raise awareness. You help promote a cause that will promote the health and health care of people across Canada. Champions can be individuals or organizations, and formal registration is not required.
